I've had that happen too! Frustrating! I think the mixture needs to reach a hard softball stage if that makes sense.. what I've done is to cook the mixture a couple minutes longer.. I have a little bowl of cold water next to the stovetop while I make the fudge and I use a spoon to drip a drop of mixture in the water and touch the drop to see how firm it is .. you want it to be relatively firm before removing the mixture from the heat and adding the butter and vanilla.. it happens to me that I'll think the drop is firm enough and next thing you know, it's not, and you end up with caramel instead of fudge
