My mom's excellent fondue recipe.
Steps

Mix together milk, mayo, Worcestershire sauce, mustard, cheese, and seasonings in 4 quart pot.

Cook over low medium heat, stirring regularly (so cheese doesn't burn to bottom of pan) for 20 minutes or until cheese melts and mixture is hot.

Preheat oven to 325°. Cube bread and cook on cookie sheet 1/2 loaf at a time. Cook for 5 minutes, stir up, and cook another 3 minutes.
